Patient information
Data protection policy
Your information should be handled with care and used for a clear purpose. This page explains how to raise a data protection question or exercise your rights. Our privacy policy describes the information used for care, bookings, enquiries and optional website measurement.
Information used for your care
Care and administration can involve identity and contact details, appointment information, clinical records, documents and relevant payment or funding information. Health information needs particular care. The reason for using information, and the people who need it, depend on the purpose involved.
- Ask the clinic if you are unsure why a detail is needed or who may receive it.
- Tell us when your contact or funding information changes so records and communications can be corrected.
- For someone else's records, we need to establish their permission or another appropriate authority before disclosing information.
Access, corrections and other rights
Send data protection requests to info@nexusphysio.co.uk, or contact the clinic if email is unsuitable. Explain what you need without sending identity documents or extensive medical information in an initial message. We may need to verify identity and clarify the request before releasing records.
- You can ask for access to your personal information and correction of inaccurate details.
- Other rights, including erasure, restriction or objection, depend on the circumstances and applicable legal conditions.
- A request to delete information does not automatically remove records that must be retained for care, professional or legal reasons.
Retention, sharing and concerns
Retention periods depend on the type of record and the reason it is held. Contact the clinic for the period or criteria relevant to your information. The privacy and cookie policies explain optional analytics, advertising measurement and the website assistant separately from your clinical records.
- For a misdirected message or suspected inappropriate disclosure, contact the clinic promptly with the minimum information needed to explain the concern.
- If a concern remains unresolved, you can contact the Information Commissioner's Office. You do not have to give up your right to raise a data protection complaint.
